Your health information is personal. Aligned Therapy Collective is committed to protecting your privacy and

maintaining the confidentiality of your records. We are required by law to protect your PHI, provide you with this

We may use and disclose your information for treatment, payment, and health care operations. Examples

include providing therapy, consulting with supervisors or other providers, processing payments, and managing

practice operations. We may also contact you with appointment reminders and treatment-related

communications.

Disclosures Requiring Your Authorization

Most uses and disclosures of psychotherapy notes, marketing communications, and any sale of your PHI

require your written authorization. You may revoke your authorization in writing at any time, except where action

has already been taken.

Disclosures Permitted or Required by Law

We may disclose your information when required or permitted by law, including for suspected abuse or neglect,

court orders, health oversight, law enforcement, workers' compensation, and to prevent a serious threat to

health or safety.

Family and Others Involved in Your Care

With your permission, we may share relevant information with family members or others involved in your care or

payment for your care.

Your Rights

You have the right to request restrictions on certain disclosures, request confidential communications, inspect

and receive copies of your records (excluding psychotherapy notes), request amendments, receive an

accounting of certain disclosures, receive notice of a breach, and obtain a paper or electronic copy of this

Notice.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Aligned Therapy Collective at alyssa@alignedtherapycollective.com or reach out to BBC (CA) or BHEC (TX) or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

Changes to This Notice

We reserve the right to update this Notice at any time. The current version will be available upon request and

through the client portal.
